Our agency has submitted a proposal to receive grant funds to conduct a study of the reliability (test-retest and interrater reliability) and validity of a medical record audit tool specifically aimed at home care patients in pain. The audit looks at the documentation of the assessment of pain, the documentation of the relief obtained from the treatment measure provided and the client satisfaction with the outcome. Little work has been done in the area of pain research in community based settings. Many individuals have expressed that their interest in assisted suicide stems from their concern about unrelieved pain. Additionally, with a recent case in which a client sued successfully because of unrelieved pain, home care agencies have cause to be concerned if they are not effectively documenting the relief of pain. After testing the audit instrument we hope to launch a state-wide study of the management of pain through community based, home care agencies. Jamie: Thanks for sharing this web site with me. I wonder if we may use this bb to share info on the pain study we well be doing? Pain is one of the top two concerns at end-of-life.

I recently visited an old friend who, since 1991, had seen four relatives die excruciating deaths--two from cancer, one from long-term liver failure, and one from Altzheimer's. She was fed up with a medical system that offers continual hope and refuses to accept the inevitability of failure--all at considerable profit to the system and great suffering for the patients and their families. She would strongly agree with Mickey's comment that our society does not deal well with the reality and inevitability of death. I wonder to what extent the cause is the infection of the general public with the medical profession's values and hubris.
