WHISTLEBLOWER PROTECTION REPORT
This Whistleblower Disclosure Report (“Disclosure Report”) has been created by WNET and its affiliates (collectively, the “Corporation”) to enable WNET trustees, officers, employees, volunteers, interns, and the WNET contingent workforce (“WNET Workforce”) to disclose anonymously to the General Counsel and the Chief Compliance Officer:
- any activities, practices or acts that are believed to be unlawful or illegal;
- any activities, practices or acts that are believed to be in violation of grant requirements;
- any activities, practices or acts that are believed to be inconsistent with the Corporation's policies or procedures; and/or
- any activities, practices or acts that are believed to be unethical or inappropriate.
An individual who files a Disclosure Report will be subject to the terms of the WNET Whistleblower Protection Policy.
WNET will treat all disclosures made in this Disclosure Report as confidential and privileged to the fullest extent permitted by law. WNET will exercise particular care to keep confidential the identity of the person filing this Disclosure Report, in accordance with the policy and procedures set forth in the WNET Whistleblower Protection Policy. The Law Department will investigate all credible disclosures and ensure proper follow-up is conducted.
Please complete this Disclosure Report to the best of your ability. Individuals may include their name or may file the Disclosure Report anonymously.
